Nurse Practitioner - Geriatrics, Primary Care

Become part of a collaborative team of mobile medical providers dedicated to improving the lives of seniors. As a Geriatric Nurse Practitioner, you'll play a vital role in providing high-quality primary care to residents in long-term care facilities.

This is your opportunity to build your expertise in geriatric care while managing residents with complex needs. You'll work alongside a dedicated team to ensure every resident receives the best possible care.

Responsibilities:

• Provide comprehensive primary and preventative care services for up to 50 residents.
• Manage residents with acute changes in condition.
• Collaborate on advanced care planning and chronic disease management.
• Prescribe medications, including controlled substances, in accordance with CO Law.
• Order, review, and follow up on treatments and diagnostic testing.
• Communicate effectively with nursing staff and collaborate with primary care physicians.

Qualifications:

• Current, unrestricted NP license in the state of Colorado (Required).
• Board Certification in Gerontology (Required).
• DEA Licensure (Required).
• Minimum 1 year of clinical experience in adult or geriatric specialties (Required).
• Minimum 1 year of experience working in a long-term care setting (Required).
• Ability to work with an EMR.
• Ability to manage patients with advanced illness and multiple chronic conditions.
• Ability to work within an interdisciplinary care team to coordinate patient care.

Competitive Benefits:

• Competitive salary range: $110,000 - $125,000 annually.

Work Schedule:

Full-time, Monday-Friday, 9:00 am - 5:00 pm.
